A patient’s hospital experience would be able to improve further if there was an application that consolidated their care plans and enabled the information on their needs to be shared across different carers and settings, with the patients’ consent.
The staff at Alexandra Hospital is exploring the idea that an app might be able to help the elderly patients once they have been discharged from the hospital and have to be taken care of at their homes.
At the Alexandra Hospital, the staff is being challenged to look for a model where the patients are looked after by only one team within the hospital, and their care after the discharge can be integrated in the community.
Last year, in October, a 3-day workshop was held and attended by almost 30 participants that included staff from the care team of hospital and also community partners.
The chief operating Officer at the Ministry of Health, Mr. Lim Cher Wee, who is chief operating officer of the Health Ministry’s Office for Healthcare Transformation for Healthcare transformation, while talking about the idea behind the app said that it allowed teams in the community to coordinate better the holistic care that was required for the patients need.
Continuing with the explanation, Mr. Lim said that the Healthcare is a very complex practice centred around the humans, and it delivers only one patient at a time.
He further added that it is essential to pay heed to the demands of the patients along with their communities, as most often they are in a better state to describe their needs of care, and also imagine the ideas that may help in addressing such needs.
